Silverchain is hosting a series of interactive CPD sessions to support integrated care across hospital, home, and community.Join our health care experts to expand your knowledge on preparing patients and families for palliative care, understanding symptom control and support systems, what services are available, and the best referral pathways. This free accredited session provides 1.5 CPD hours and includes refreshments.
The presentation will be led by Silverchain Medical Director Palliative Care WA, Dr Fiona Findlay, and Silverchain General Practitioner with special interest in Palliative Care, Dr Susan St Clair. They will share their knowledge and support you to work through different case studies during the session.

Learning outcomes covered include:
- Distinguish between palliative care options available in WA and Silverchain to support patients with their preferences.
- Understand the referral pathways for home based palliative care and requirements to optimise outcomes for patients choosing to die at home.
- Develop a better understanding of community based services and resources available for patients and families.
- Outline the role of the General Practitioner in identifying and providing care for patients, along their illness trajectory to the end of life.

Our expert presenters
Dr Fiona Findlay - Silverchain Medical Director, Palliative Care WA
With a wealth of palliative care experience, Dr Findlay leads a team of specialists, general practitioners, and registrars to provide medical oversight over Silverchain’s in home palliative care service. She’s also a Palliative Medicine Consultant at Royal Perth Hospital and participates in the state wide outreach on-call service.
Dr Susan St Clair - General Practitioner, special interest in Palliative Care
With over 20 years' experience in general practice, Dr St Clair has worked in community palliative care with Silverchain since 2011, and in the Peel region since 2017. She has a Clinical Diploma in Palliative Medicine and enjoys the balance of general practice, special interest practice and involvement in GP registrar training.
