1. The person named under Booking Details (Consent Party) is the parent or other legal guardian of the player(s) registered for the event (Trainee).
2. The Consent Party is fully aware that the Trainee’s participation in the event will involve risk of injury to the Trainee.
3. The Consent Party has made all the enquiries it wishes to make about the Trainee’s participation in the event, about the risks to the Trainee in doing so and about the Trainee being an appropriate person to do so.
4. The Consent Party requests Sydney Swans Limited (SSL) to allow the Trainee to participate in the event and in consideration of SSL agreeing to allow the Trainee to participate in the Program, the Consent Party:
a. Waives all and any rights or claims the Consent Party in its own right, and on behalf of the Trainee, all and any rights or claims the Trainee has or may in future have in relation to the event, the Trainee’s participation in the event or the way in which the event is or is not conducted;
b. Indemnifies SSL and each of its officers and members and others associated with SSL from and against all and any claims, actions or demands by the Trainee or any person on behalf of the Trainee or the Consent Party or any other person arising out of or in relation to the Trainee’s participation in the event or the way in which the event is or is not conducted.
5. The Consent Party shall permit SSL to photograph and video the Trainee individually, or as a member of a squad of players and staff of SSL, provided such photographs are for the use as official photographs of SSL.
6. The Consent Party shall permit SSL in taking, or arranging to be taken, video and television footage of the Trainee for use in:
(a) television programmes to be screened on Fox Footy or as other television programmes and on other television channels decided by SSL;
(b) training and promotional activities decided by SSL. Consent is also given to SSL owning the television or video footage so taken and the intellectual property in it.
7. The Consent Party acknowledges that SSL will not provide refunds for the event under any circumstance including but not limited to injury, sickness, registration error or wet weather cancellations.
