By purchasing this ticket attendees at the Fit For Sport Training sessions should recognise:
A. Conditions in and around the training facilities in use on the day.
B. The nature of certain activities all present certain reasonable and foreseeable risk or injury.

Our clinics and athletic programs are designed for the targeted age groups. It is required that any potential attendee that has sustained an injury prior to the clinic date please declare this prior to the clinic / training sessions commencing.

By purchasing this ticket I hereby assume the risk of all conditions or occurrences which may be encountered, and waive all claims for damages, injury or loss to my person and/or property which may be caused to the participant (mentioned above) during their participation in the speed & agility clinic / training sessions.

In completing this form, I claim, to the best of my knowledge, that my child does not have an illness (medical or physical), that would preclude safe participation in this program. I will update my registration form and notify the instructor if there is any change to the medical information provided.

Refund Policy:

i. Refunds can only be issued in full (provided the clinic has not been sold out) and only if a request to cancel has been received prior to 48hrs before the start of the clinic.
ii. If a request has been made inside 48hrs from the start of the clinic you can receive either of the following, a 50% refund or a credit towards another clinic, valid for 12 months. (this only applies if the clinic has not been sold out. if the clinic is sold out and ticket sales have stopped there is no refund)
iii. Cancellations inside of 24hrs or on the day prior to the start of the clinic will be issued with a credit towards another clinic valid for 12 months.
v. If you fail to turn up on the day without notice prior to the start of the advertised clinic time, all booking fees will be forfeited and no credit will be issued.
